Buzzardball Waiver & Release of Liability
Please read this waiver carefully before completing registration.
1. Permission to Participate
I am the parent or legal guardian of the above-named participant. I give my permission for my child to participate in all programs, practices, games, camps, clinics, events, and activities operated, organized, or sponsored by BUZZARDBALL Youth Basketball (“Buzzardball”).
2. Assumption of Risk
I understand and acknowledge that participation in youth basketball and related activities involves inherent risks, including but not limited to:
* Physical contact with other participants
* Running, jumping, and rapid changes in direction
* Use of gymnasiums, courts, equipment, and facilities
* Risk of minor or serious injury, illness, or property damage
I understand that these risks exist even when reasonable safety precautions are taken. I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation.
3. Medical Fitness + Responsibility
I certify that my child is physically and medically able to participate in Buzzardball activities. I acknowledge that it is my responsibility to:
* Inform Buzzardball of any medical conditions, allergies, or limitations
* Provide appropriate medical insurance coverage for my child
* Monitor my child’s health and fitness throughout the season
I understand that Buzzardball does not provide medical insurance for participants.
4. Emergency Medical Authorization
In the event of an emergency and if I cannot be reached, I authorize Buzzardball coaches, volunteers, staff, or agents to obtain emergency medical treatment for my child as deemed necessary for their safety and well-being.
I understand that I am financially responsible for any medical care or transportation provided.
5. Release of Liability (Including Ordinary Negligence
To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue BUZZARDBALL Youth Basketball, its officers, directors, employees, independent contractors, volunteers, agents, facility owners, and representatives from any and all claims, demands, causes of action, or liabilities arising out of or related to my child’s participation.
This release includes claims arising from ordinary negligence, but does not apply to gross negligence or willful misconduct, to the extent prohibited by law.
6. Indemnification & Hold Harmless Agreement
I agree to indemnify, defend, and hold harmless Buzzardball and its representatives from any claims, damages, costs, or expenses (including attorney’s fees) arising from:
* My child’s participation
* My actions or omissions
* Any claims brought by or on behalf of my child
7. Facility & Equipment Acknowledgement
I acknowledge that Buzzardball activities may take place at shared or third-party facilities, and that conditions may vary. I accept responsibility for myself, my child, and any family members or friends attending a Buzzardball game or practice for risks associated with:
* Playing surfaces
* Equipment use
* Facility layout and conditions
8. Photo & Media Release
I grant Buzzardball permission to use photographs, video, or audio recordings of my child taken during activities for promotional, educational, or marketing purposes, including social media and websites, without compensation or further approval.
9. Governing Law & Severability
This agreement shall be governed by the laws of the State of California. If any portion of this waiver is found unenforceable, the remaining provisions shall remain in full force and effect.
10. Acknowledgement & Signature
I have carefully read this waiver, understand its contents, and sign it voluntarily. I understand that by signing this document, I am giving up certain legal rights on behalf of myself and my child.
